The Post 16 Team are keen to engage with parents/carers, to gain an insight of your experiences with Post 16 Transition. The Post 16 Team has been created to collaborate with Post 16 providers to improve outcomes for young people in Medway. An important part of this work, is engaging with parents/carers and understanding your experiences. Proactively identifying areas of good practice, challenges, and barriers from lived experiences is key to initiating change and developing strategies to improve.
In preparation for our meeting on the 1st October 2024. We would be grateful if you could complete the attached Microsoft Form that can be found in the above attachment on page 2. This will allow the team to gain an initial insight to inform and lead our discussions.
